REQUIRED ONLY FOR NEW STUDENTS: *Student’s Current Immunization Record *Student’s Birth Certificate *Social Security Card (if available) *Proof of Address (utility bill – water, electricity, gas, or lease agreement) *Parent/Guardian Photo ID (guardianship paperwork) * Withdrawal/Report Card from Previous School
