ENFERMEDAD ACTUAL: Paciente de 23 años sin alergias medicamentosas conocidas. ANTECEDENTES PATOLÓGICOS: - Pieloplastia robótica izquierda en 2015. MOTIVO DE CONSULTA: Remitido de la Base de Rota por persistencia algia fosa lumbar izquierda desde la intervención quirúrgica. Aporta Renograma: Insuficiencia de Ostium izquierda, función relativa izda 44.74 % Se solicita por el urólogo de la base dilatación endoscópica. Solicito TAC ABDOMINOPELVICO descarte patología obstructiva externa. Se entrega ci endopielolitotomía endoscópica en castellano para su traducción. EXPLORACIÓN FÍSICA: - ORIENTACIÓN: Consciente y orientado - ASPECTO GENERAL: Buen estado general - NUTRICIÓN: Buen estado de nutrición - HIDRATACIÓN: Bien Hidratado/a - FRECUENCIA RESPIRATORIA: Eupneico/a - CABEZA Y CUELLO: Carótidas palpables no ingurgitación yugular, no adenopatías. - A.C.: Rítmica sin soplos. - A.P.: murmullo vesicular conservado - ABDOMEN: Blando, no doloroso, no masas ni megalias. - MM.SS./MM.II.: No edemas, no signos de trombosis venosa profunda, pulsos presentes. - NEUROLOGÍA BÁSICA: Sin focalidad. HISTORY OF THE PRESENT ILLNESS: 23-year-old patient with no known adverse drug reactions. PERSONAL HISTORY: - Left robotic pyeloplasty in 2015. REASON FOR CONSULTATION: Referred from U.S. Naval Rota due to persistence of pain in left lumbar region since surgical intervention. Brought renogram study: Left ostium insufficiency, left relative function 44.74% ABDOMINOPELVIC CT SCAN in order to rule out external obstructive pathology. Endoscopic endopyelotomy in Spanish handed over in order to be translated. PHYSICAL EXAMINATION: - ORIENTATION: Conscious and oriented. - GENERAL APPEARANCE: Good general appearance. - NUTRITION: Good nutritional conditions. - HYDRATION: Well hydrated. - RESPIRATORY RATE: Eupneic. - HEAD/NECK: Palpable carotids, no jugular ingurgitation, no adenopathies. - H.A.: Rhythmic, no murmurs. - L.A.: Vesicular breath sounds preserved. - ABDOMEN: Soft, non-painful, no masses nor visceromegaly. - Upper Limb/Lower Limb: No edema, no deep vein thrombosis signs, pulses within normal. - BASIC NEUROLOGY: no signs of neurological foci.
