LOCATION: The Salvation Army · 736 W. Main Street, Lexington, KY 40508 REQUIRED DOCUMENTATION: All documents must be in the name of the applicant. 1. A picture I.D. of the applicant – parent or guardian registering the child(ren) -- No social security cards 2. Parent or Guardian must provide a birth certificate and one document that verifies the child(ren) resides in Lexington, Fayette County Immunization Records, School Records, lease or Snap letter. *Official court documentation required if applicant is not parent.* 3. Proof of all monthly income and source of income from all household members. (Paycheck stubs, a letter of verification from SNAP, Social Security, SSI, proofs of child support)

4. Verification of Fayette County address within the last 60 days (lease or utilities) Must have name of applicant and a Fayette County current address.

In order for an applicant to be eligible for The Salvation Army Angel Tree assistance, he/she must NOT have applied for Christmas Assistance elsewhere.
